REGISTRATION NOW OPEN FOR 2013-2014 SEASON

MOHA is now accepting registrations for the 2013-2014 season.  Please click on the below link and follow the promps to register your child.  

If you are a new registrant to MOHA you must fill out a manual registration form and submit it to the MOHA office with payment, a copy of the participants birth certificate or passport and a copy of the parent/ guardian's driver's licence as proof of address.

If your child is Minor Peewee aged and older be sure to read the message and review the attachment pertaining to MOHA's mandatory concussion program which will appear when you select those age groups.

REGISTRATION FEES:

Early Registration Fee: $525 available until June 20, 2013

Late Registration Fee: $595 June 21-August 15, 2013.

Registration will be closed after August 15, 2013. A number of divisions fill up early, it is recommended that you register your child as soon as possible.

REGISTERING:

MANUAL REGISTRATIONS: Will begin being accepted at 1pm on Tuesday April 9, 2013. If you are paying by cheque please ensure that you bring a cheque for the full registration amount made payable to the Minor Oaks Hockey Association. Post Dated cheques will not be accepted.

SPECIAL CONDITIONS

By registering with the MOHA, the applicant and their parents or guardians acknowledge that they have all read the MOHA Code of Conduct and they each agree to abide by this Code of Conduct as a MOHA member. Furthermore they also agree that should they be guilty of a violation of the MOHA Code of Conduct they are subject to and will abide by MOHA's Disciplinary Standards and decisions.

By registering with the MOHA, the applicant and their parents or guardians acknowledge that they are opting-in to receive email communications from MOHA and that any and all email communications from MOHA to them does NOT constitute SPAM or unwanted email communications.

The Minor Oaks Hockey Association has also mandated a compulsory concussion testing and Return-To Play Protocol. Every player from Minor Peewee to Juvenile is required to take a baseline concussion test before they will be allowed to step on to the ice. More details will be available on the MOHA web site under the Administrative tab- "Trainer/Health/Concussion"
